What is AffirmHD?

AffirmHD (Affirm Health Direct) is Pataskala, OH’s first Direct Primary Care practice. We are a Family Medicine office that provides direct primary care services within the scope of Family Medicine to patients of all ages. We ascribe to the vision of caring for people from “the cradle to the grave”

Is concierge medicine the same as Direct Primary Care?

The terms are often used interchangeably and essentially refer to the same principles with some differences. Concierge medicine practices often times will bill insurance as well in addition to the direct payment from patients and often times have higher associated membership fees.


Direct Primary Care is a healthcare model focused on putting the patient first. That means treating them when they need it, never rushing patients through appointments, and being proactive with healthcare treatment plans. The doctor-patient relationship is just as the name suggests – direct.


Direct Primary Care clinics don’t accept insurance, rather they choose to work directly with the patient, providing wholesale labs, deeply discounted prescription prices, and other costs savings along the way.

What sets AffirmHD apart from other practices?

We have cut the red tape of bureaucracy. The restrictions that typically leave physicians in a traditional healthcare setting buried in paperwork are lifted through Direct Care, allowing us to spend our time where it really counts: with patients. In keeping out any interference with the sanctity of the patient-physician relationship, we have returned family practice medicine to its root values of clinical excellence for and compassionate knowledge of every patient. In every aspect of family healthcare, and custom healthcare, we promise accessibility, convenience, and affordability.

Will I benefit from AffirmHD if I don’t require frequent medical attention?

Of course! We might argue that everyone benefits from a service tailored directly to them. That’s why we offer truly custom healthcare. Our practice is devoted to the maintenance of your good health, to the enhancements of your daily experience that only physical well-being can bring. We’re here for your reactive needs. However proactive healthcare is important, and we’re here for that, too. Besides no one can 100% of the time predict precisely when they will need access to their doctor. For those unpredictable times we will be here for you.


At AffirmHD our aim is to give you time. Large amounts of precious, irretrievable time is given to you to allow you to get your needs met and to be heard. At every visit, for whatever reason, we work to  allow enough time for thoroughness, to answer your questions and address the needs of the moment. Then over time we’ll build a comprehensive, intimate knowledge of your health that allows for an invaluable understanding of you and your needs.

Does AffirmHD accept health insurance?

We do not bill insurance for the care we provide in our office. We welcome patients to our practice not based on their insurance carrier. Our members are more to us than insurance units.


Perhaps the most critical distinction in our model of care centers on a direct, personal relationship between you and your doctor. That directness carries over to our business model as well. While the payments offered by an insurance company might apply to the fixing of a damaged fender on your automobile, we desire to avoid the intrusion of any insurer into your family’s care in our practice.


We forgo insurance payments in order to save our patients from the arbitrary, intrusive decisions and costs that can inevitably come with the involvement of third-party payers. Furthermore, this direct fee-for-services arrangement frees us from the typical contractual agreements that prevent physicians from offering common sense and affordable care including wholesale prices on laboratory tests, imaging, and medications.


If you have insurance through a third party,  you are still welcome to use the coverage for services offered outside of our practice at the lab, the local pharmacy, the hospital etc.

As a patient of AffirmHD, will I still need health insurance?

Yes. While we do not want the intrusion of third party payors in our physician-patient relationship in our model of care, we are not anti-insurance. We feel health insurance has a place in the healthcare field. As such we do recommend our patients continue a major medical plan with a high deductible and health savings account to ensure financial help should hospitalization or referral to a specialist be necessary. While a lot of things in primary care are predictable and we feel can be planned, there are major emergencies or health issues that can come up for which a major medical plan can be very helpful. Please note that we do not provide information so that you can be reimbursed by your insurance provider however for our office visits.

If I have Medicare, can I still join AffirmHD?

We would love to care for our seniors ages 65 and over covered under Medicare, however per Medicare rules we are not allowed to do so at this time.  In the future when this changes, as we anticipate it possibly will, we can notify those who are willing to be on wait list for membership.

Is my membership to AffirmHD tax deductible?

Please confer with your tax consultant to clarify tax consequences in your particular circumstances. There is currently legislation awaiting further action in Congress H.R 6199 so there might be changes in the tax code in regards to use of Health Spending Accounts (HSA) which may offer some tax advantages. Contact your legislators and encourage them to vote to allow you to spend your income towards this and other forms of valid healthcare spending.

What happens if I need to go to the hospital or see a specialist?

In providing superb care with unprecedented sorts of access, we seek to prevent or reduce hospitalizations and specialty referrals. In instances where those types of care are required, we will certainly continue to work with your hospitalist during your hospital stay as well as work closely with any specialist. In some cases, depending on the level of care and the sorts of expertise required, we may rely on those specialized caregivers to manage your treatment.

Can children become members of AffirmHD?

Yes. We are happy to accept the children (birth to 18 years of age) of our enrolled clients. At this time, we are not able to provide routine vaccinations for young children but we will happily work with you to arrange for these.

Can I contact my physician after hours? And how?

By all means, yes! Because illness and injury do not respect regular office hours, you may call your physicians after hours. We are even willing to consider arranging for home visits within a limited area, schedules permitting. If you prefer, you may also contact your physician via email or text messages. It’s how we do custom healthcare.


We do recommend you call for truly urgent needs that cannot wait to be addressed later rather than email or text.


Sometimes you may have a truly emergent need where immediate safety, life or limb is at stake. For these types of immediate needs we recommend you call 911 immediately.

What if I need medical attention while I’m away from home?

We provide such care – yet another freedom we enjoy together as a result of AffirmHD’s’ independence from the restrictions of third-party payment. Because we know you well, we can streamline your care when you’re sick away from home. Many illnesses can be diagnosed and treated with a simple conversation by phone, email, text or webcam. We’ll work with you to locate the nearest pharmacy and order the medications most appropriate for your circumstances if needed.

Are my medical records ever shared with insurance carriers or pharmaceutical companies?

We value your privacy and work to maintain a high standard of privacy. We are never obligated to provide any third party with a copy of your records unless you specifically ask that we do so or it is part of providing further care as in the case of referrals to other professionals you have consented to already.

What if I change my mind after I enroll?

A 30 day written cancellation notice is required to cancel your membership. All fees incurred during previous periods of active membership are due once cancellation notice is received. Your enrollment fee is non-refundable. Refunds will not be provided for cancellations made after payment for a month has been made as we will continue to be available for the rest of the month after cancellation is made.

How will I know if the doctors of AffirmHD are right for me?

Come get to know us! Schedule a brief unconditional getting to know you visit to discuss your healthcare needs. It’s the only reasonable way to begin what we trust will be a long, productive, and healthy relationship. Please be aware that we will not be prescribing medications or providing any procedures during a getting to know you visit since these visits are just to introduce you to our practice. There are limited slots available for these visits per day so we can leave time to take care of our member patients.
